Paperwork Required for Small Sided RefsPrintable

To participate in a NSYSA Program, you must first register and create a profile via Logon Register.-> Please be sure that you affiliate with the Club you will be associated with, vice NSYSA. Submittal of all documents and entering of personal information shall be via the profile record. Please note that this information is Privacy Act Information and must be treated responsibly.To participate in a NSYSA Program, there may be a requirement for a specific form, document, etc. that you must provide to the NSYSA (we call it Paperwork). Generally, you can provide the necessary documents to the District via the U.S. Mail OR you can provide it electronically via Member Paperwork.->

Keep in mind that the required Paperwork can change from year to year, as many of the requirements are imposed upon the NSYSA by other organizations (e.g. the U.S. Government, Washington State, WSYSA, etc.).

That said, the following Programs have the identified requirements:


Small Sided Youth Referees

Youth Referees (those under age 18) are required to have on file the following documents:

    • W9
      The required W9 form can be generated directly from the IRS-> site
      From this screen, simply click on W9 and then fill out the W9 Form on your screen using the instructions provided.{See W9 Sample below}
      Next, print out the document and sign and date the applicable blocks in Part II.

      Send the completed W9 form to the District following the directions below concerning Completed Forms/DocumentsGoing

  • SSN on file in Members Profile
    Referee's SSN must be on file in the Members individual Profile.

Adult Referees (those age 18 and above) are required to have on file the following documents:

  • W9
    The required W9 form can be generated directly from the IRS-> site
    Simply fill out the W9 Form on your screen using the instructions provided. {See W9 Sample in Youth Referee Section above}
    Next, print out the document and sign and date the applicable blocks in Part II.

  • WSYSA Risk Management Clearance
    You are required to obtain and maintain a valid, up to date Risk Management Clearance through WSYSA.

    To apply for either an initial or renewal of an existing Risk Management clearance, we recommend that you do so via your Club/Association website if they have one. If not, then please follow the instructions provided by WSYSA here->

    Note:
    For most Clubs, Small Sided Referees will register as Volunteers. Check with your Club SS Referee Assignor to confirm which category, volunteer/non-volunteer, to use during registration.

    During registration within Affinity, identify your association for the Club you are affiliated; vice NSYSA.


For New (adult - age 18>) Referee/Volunteer RMA Registration

1. Choose "Registration."

2. Create an account.

3. For most Clubs, Small Sided Referees will register as Volunteers. Check with your Club SS Referee Assignor to confirm which category, volunteer/non-volunteer, to use during registration.

4. Please choose your Club for which you are affiliated with.

5. Finish the registration process.

6. As a small sided referee and a volunteer, you will not need to pay the $6 fee of registration.

7. As a small sided referee and a non-volunteer, you will need to pay the $6 fee of registration.

You can later review the current WSYSA RM Status after registration, within your NSYSA profile by going to Member Risk Management

  • WSYSA Risk Management Clearance
    You are required to obtain and maintain a valid, up to date Risk Management Clearance through WSYSA.

    To apply for either an initial or renewal of an existing Risk Management clearance, we recommend that you do so via your Club/Association Affinity website if they have one. If not, then please follow the instructions provided by WSYSA here->

Completed Forms/Documents

In order to send a completed form/document electronically to the NSYSA Website, you'll first have to have a digital image of the Paperwork saved in a file as a PDF, JPG, or TIF file (PDF preferred).

Once you have scanned a copy of any of these complete forms/documents noted above (W9, etc.) to your PC, the resultant file can be uploaded to NSYSA on the website via Member > Paperwork > Upload 

If you do not have the capability to send your completed documents/forms electronically to NSYSA the options below is available.

1. Contact your Club Referee Assignor for assistance in forwarding documents.

2. Lastly, Registered mail is best. The process of using the United States Postal Service to deliver these documents may result up to 45 days delay. Please mail the completed forms/documents
